Pre-Registration Process

  1. Have a diagnosed qualifying debilitating medical condition

  2. Call our Offices at (201) 288-6781 and ask for Dillon

  3. Fax your medical records to our offices at 201-288-4575, attention Dillon

  4. When the records have been received Dillon will give you a call to schedule the initial appointment.

  5. Bring required documentation to initial visit. 

Driver’s license (non-driver ID and county card also accepted) as well as either a utility bill from the last 90 days with your name on it, (gas, water, electric, phone, cable etc) or any correspondence with the IRS or the NJ state tax office within the last year. 

 * Fill out the REGISTRATION FORM and bring it to your first time appointment.

Post Appointment Process

  1. Wait for an email from the state verifying that they have reviewed your application. This will take between 48 hours and 3 weeks.

  2. Make your payment on the website https://njmmp.nj.gov. If you are having trouble you can click HERE to be taken directly to the payment page.

  3. Wait for your card to come in the mail. This will take between 7 and 10 business days

  4. When you have received your card, schedule a consultation appointment with the dispensary of your choosing.

 

Debilitating Medical Conditions:

A physician must certify that a patient has an approved debilitating medical condition to participate in the Medicinal Marijuana Program. Approved debilitating medical conditions include:

  • Amyotrophic lateral sclerosis
  • Anxiety
  • Chronic pain related to musculoskeletal disorders
  • Chronic pain of visceral origin
  • Migraine
  • Multiple sclerosis
  • Terminal cancer
  • Muscular dystrophy
  • Inflammatory bowel disease, including Crohn’s disease
  • Terminal illness, if the physician has determined a prognosis of less than 12 months of life

    The following conditions apply, if resistant to, or if the patient is intolerant to, conventional therapy:

  • Seizure disorder, including epilepsy
  • Intractable skeletal muscular spasticity
  • Glaucoma
  • Post-Traumatic Stress Disorder (PTSD)

The following conditions apply, if severe or chronic pain, severe nausea or vomiting, cachexia or wasting syndrome results from the condition or treatment thereof:

  • Positive status for human immunodeficiency virus
  • Acquired immune deficiency syndrome
  • Cancer
